Personal Care Assistant - On-Call; Part-time; Full-Time

 This position is responsible for providing personal care and designated health-related services to enhance and/or maintain each tenant’s physical and emotional well-being. This position will be responsible for assisting tenants with personal cares, treatments per each tenant’s individualized care plan under the direction of the RN Memory Care Manager, administration of medications, performing specified home services such as laundry services, cleaning, meal service, preparing and/or facilitating resident social and recreational activities, monitoring and reporting any changes in tenant conditions to applicable staff, serves as a mandated reported and promptly reports any concerns, promptly responds to tenant requests.
